Medical Mileage Reimbursement Rate Increases in 2025
As of January 1, 2025, the reimbursement rate for medical mileage for workplace injuries—such as transportation to and from a treating physician, the pharmacy and physical therapy—will increase to 70 cents a mile, according to the California Workers' Compensation Institute.
California's Workers' Compensation medical mileage rate is tied to the Internal Revenue Service's published mileage rate, which is based on a yearly study of the fixed and variable costs of operating a vehicle.
Complete your medical mileage form accurately and submit it to your adjuster periodically, making note of the date. Also, send a copy to our office, and keep a copy for your records. 2026 update
For travel from January 1 through June 30, 2026, the medical mileage rate is 72.5 cents per mile. Following a mid-year increase by the IRS, the rate rose to 76 cents per mile for travel on or after July 1, 2026. The rate that applies is the one in effect on the date you drove, so keep track of when each trip took place.
Tips for getting reimbursed
- Log every trip related to your claim: treating doctor visits, physical therapy, the pharmacy, diagnostic tests, and medical-legal evaluations such as a QME appointment.
- Record the date, the address, and the round-trip miles for each trip.
- Submit your log to the claims adjuster regularly instead of saving it all for the end of your case, and keep copies of everything you send.
